TEMPERATURE LOG

Temperature logs must be kept for all freezers, refrigerators and dry storage areas. They must be updated a minimum of twice per week. You'll need to retain each month’s log for review during your agency’s site visit. Download and print a temperature log: Temperature Log

REPORT A CHANGE

Report any changes about your program to Carl Rue at carlr@shfbmadison.org or 608-216-7207 as soon as possible! Changes or addition may include phone numbers, email addresses, billing address or even a new director or contact for your program.

FoodShare Wisconsin

Your clients may be eligible to receive FoodShare, Wisconsin’s name for the federal Supplemental Nutrition Assistance Program, formerly known as food stamps. FoodShare puts healthy, nutritious food on the table for our low-income families, children and seniors. Plus,the USDA estimates every $5 spent with FoodShare benefits generates $9 in local economic activity.

Help your clients learn if they may be eligible to receive FoodShare by telling them about ACCESS at www.access.wi.gov, Wisconsin’s online webtool, where they can complete a quick eligibility screen and apply for FoodShare, Health Care, Family Planning Waivers and Child Care benefits. Those already receiving benefits can make changes to their accounts and manage their benefits.

2-1-1


Make sure your clients have this phone number handy at all times. By dialing 2-1-1 (like 9-1-1), they can learn about free community resources such as assistance with food, housing and utilities in addition to a range of other services.
