Become a Partner AgencyThank you for your interest in partnering with Second Harvest Foodbank of Southern Wisconsin in the fight to end hunger in southwestern Wisconsin. The Foodbank provides thousands of low-income people with hot meals and groceries through 300 dedicated partner agencies. In addition, our FoodShare (formerly known as food stamps) Outreach Program assists potentially eligible people throughout 16 counties apply for benefits that help them put food on their table every month.

We welcome inquiries from any non-profit organization in our service area that provides free food to people in need including:

  • Emergency Food Providers, such as a food pantry, soup kitchen, or emergency shelter, and
  • Non-Emergency food assistance programs, such as an adult or child daycare, long term transitional shelter, or an after school youth program.

Due to a strategic decision to focus on building the capacity of our current member agencies, we are not accepting new applications for supportive living and residential programs.

PARTNER AGENCY MEMBERSHIP APPLICATION
Follow these steps to submit a new partner agency membership application:

 

  1. Review the following quick evaluation tool to find out if you qualify for membership: Quick Evaluation Tool
  2. If you haven't already, carefully review: Partner Agency Membership Criteria
  3. Download complete the Partner Agency Application. Make copies of all requested documents to include with your application.
  4. Make a copy of the completed application for your files.
  5. Mail the original completed application, member agency agreement and required documents to:

Once Second Harvest Foodbank receives your completed application and proof of your nonprofit status, we will review it and contact you to discuss the next steps in the process.
